I have still yet to really grasp the differences between all of these. I usually just go with the default of OnBoot because I have enough ram and usually trying to move things between onboot and ondemand just causes me more problems.
really though, what exactly is the purpose of each?

OnBoot: the extension will be loaded this time and every time the machine boots
OnDemand: a menu item/icon will be created to allow the extension to be easily loaded when required.
Download: the extension will be downloaded, but not loaded. The extension will need to be manually loaded using the apps browser "local" if required
Download and Load: the extension will be downloaded and loaded this time only

there is no icon/button/text in my menu am i missing something?
I remasterd TC to my needs but i can't see any of my OnDemand packages which i put into /tmp/tce/ondemand.
Is there a file i have to edit to see these packages?

You do not put extensions into tce/ondemand.
All extensions belong in tce/optional.
You use AppsAudit to select them for ondemand.
