I have installed tiny core to a usb stick fine. I want the wicd extension to be permanent and be able to install a couple of programs like netbeans and gimp without them being extensions, so i can have a complete working environment that I can backup and carry with me to work on any laptop, netbook or desktop that boots from usb.

so far the wicd extension is still on the usb key after reboot, but it needs to be reinstalled from app manager after reboot, and any changes i make to home disappear after reboot. I have tried mounting the usb key, setting the backup drive and selecting backup when shutting down/rebooting but changes are never there.

Im a little confused and would appreciate any help.

I think you need to remaster Tiny Core to have your extensions bundled in the iso.
There is a wiki page how to remaster. Or you can just use: ezremaster.tcz it's a great GUI based wizard for this task.

..or you could set wicd to be "onboot" and netbeans/gimp to be "ondemand" using appsaudit in the panel.

In order not to lose your changes on reboot you would need to check any settings files in /usr/local/etc for changes and add them to your backup if required. Note that any changes to settings files under /home/tc would be backed-up automatically.
