it appears that /usr/local/etc/gconf/schemas/gstreamer-0.10.schemas -> /tmp/tcloop/gst-plugins-good/usr/local/etc/gconf/schemas/gstreamer-0.10.schemas controls the audio target for rhythmbox (I could easily be wrong).
I tried:
--- Code: --- $ gconftool-2 -t string -s /system/gstreamer/0.10/default/musicaudiosink "alsasink device=bluetooth"

..which creates an entry in ~/.gconf/system/gstreamer/0.10/default, but this doesn't change things.
I also tried changing the same key in /usr/local/etc/gconf/schemas/gstreamer-0.10.schemas - again without success, any ideas?

After some more playing (sic) around, it seems the issue can be solved as long as things are done in the correct order.
Due to the fact that bluez and avahi (a rhythmbox dep) install dbus conf files, the init.d script for these re-starts dbus - this doesn't appear to be neccessary for bluez, but it does appear to be required for avahi.
If I load the bluez, bluez-gnome, dbus and rhythmbox extensions and then:
start dbus using the init.d script
start avahi directly (i.e. without the init.d script)
gconftool-2 -t string -s /system/gstreamer/0.10/default/musicaudiosink "alsasink device=bluetooth"
change the same key in /usr/local/etc/gconf/schemas/gstreamer-0.10.schemas
start rhythmbox
start bluetoothd directy (i.e. without the init.d script)
pair with the bluetooth headphones
..rhythmbox will play to the bt headphones without problems.
If the above is not done, then I get many errors of the form:

..which looks like it is something to do with the new GConf dep on dbus?
