The latest version seems to be from 2008?
Have you tried the espeak-ng, orca or speech-dispatcher extensions?

I haven't. It's been a while since I've looked into screen readers. Is there one already available for Tiny Core that I can try out?

Looks like only speech-dispatcher is available for 64 bit. Tried running it, and I just get an error: "setlocale: No such file or directory". Did I screw up and delete a package somewhere or something?

espeak-ng and orca are also available in the 64-bit repo.
I don’t recall that the locale needed to be set for speech-dispatcher, but you could try that.

I just double-checked:

$ espeak-ng "test text"
$ spd-say -o espeak-ng "test text"
..both work using pulseaudio (they should also work with alsa) and without a locale being set.
You will probably need gnome-session for orca to work.
